Review Reporting and Record Keeping
During the FTR a reviewer the recorder actively records all issues which have been increased. These are summarized at the last of the review meeting and a review matter list is produced. In addition, with a simple review summary report is completed. By this a review summary report answers three questions:
1. What was reviewed?
2. Who reviewed it?
3. What were the conclusions and findings?
The review summary report is normally a single page form with possible attachments. It becomes category of the project historical record and may be distributed to other interested parties and the project leader
The review issues list serves 2 purposes that are: (1) to recognize problem areas within the product and (2) to serve as and action item checklist which guides the producer as corrections are made. As an issues list is generally attached to the summary report.
It is very important to build a follow-up process to ensure which items on the issues list have been properly corrected. Unless this is done it is possible which issues raised can fall among the cracks. One approach is to assign the responsibility for follow-up to the review leader and the other one is formal approach as signs responsibility to an independent SQA group.
