Aldosterone mediates myocardial remodeling and fibrosis, as well as sodium retention and potassium loss at the distal tubules. The anti-aldosterone agent, spironolactone has been shown in RALES trial as an important neuro-hormonal antagonist in the treatment of heart failure.
