Use your imagination, but the report should include all the submitted information, except the password, and generated information from the database too. A routine response might be:

The incident# has to be extracted from the database - but not using an SQL statement. Research some other PHP-based technique to access this generated number. Use a style rule(s) to control the font for the echoed response as well as the Form page! The style can be placed at the top of the PHP file (before the PHP code section) as part of the HTML segment allowed there, or optionally for extra points in a style sheet. It must have font-family, font-size, text color and background color settings.
