Q. Implementation of the Plan in diabetes mellitus?

You have helped the client to find out an alternative solution and to develop a plan to solve the problem. Now, it has to be implemented. Motivate and support the patient and family to solve the problem or bring change in life style or behaviour.  Keep a record of  all the actions, problems faced, successful steps, change in behaviour of the client and the family. If needed re-view from time to time and modify the plan. Make client independent to handle the problem in future with the support of the family.
