Describe the types of Drugs
• Induction agent (thiopentone, fentanyl, ketamine, midazolam) • Suxamethonium (1-2 mg/kg) is the muscle relaxant of choice.
Rapid sequence induction and orotracheal intubation:
• Pre oxygenate with 100 per cent oxygen for 3-4 minutes. • Preload with 250-500ml colloid intravenously. • Induction agent + suxamethonium. • Cricoid pressure applied. • Direct visualisation of vocal cords and tracheal intubation. • Inflation of cuff until sealed. • Confirmation of end-tidal CO2 and chest auscultation with manual ventilation. • Cricoid pressure released. • Secure tube at correct length. • Connect patient to ventilator (see default ventilator parameters). • Ensure adequate sedation ± muscle relaxant. • Consider insertion of a naso/oro-gastric tube. Required by the majority of ICU patients. • Chest X-ray. • Confirm blood gas analysis and adjust FIO2 accordingly
