Assignment -
Write response on Neurological that might occur for that body system which does not have to be something that can be performed in the medical assisting office. The response post must be in detail.
1. Gather all necessary equipment which may include percussion hammer, safety pin or sensory wheel, peppermint or alcohol prep, cotton ball, solid objects such as keys or coins, tuning fork, flashlight or penlight, tongue blade, and nonsterile gloves.
2. Wash hands and follow Standard Precautions.
3. Introduce yourself to the patient.
4. Identity the patient using two identifiers
5. Explain the reasoning for the procedure while being aware of the patient's concerns.
6. While obtaining a health history from the patient, pay attention to the patient by observing the "patient's ability to manage cognitive functioning" (Wilburta Q Lindh, 2018). This will include:
a. Are they oriented to person, place and time?
b. Any changes in level of consciousness such as periods of confusion or lethargy and history of coma?
c. How is the patient's attention span? Are they able to concentrate on your questions and respond?
d. How is their Judgment? (What would you do if your house caught fire?)
e. Any changes in mental function and mood?
f. Is or has the patient experienced any headaches? Note frequency, location, severity, duration, and nausea.
g. Is or has the patient exhibited any visual changes such as double vision?
h. Is or has the patient experienced any changes in hearing?
i. Any dizziness or vertigo?
j. Any abnormal sensations such as numbness, tingling, or burning?
k. Any weakness? Include location and duration.
l. Any pain?
m. Describe their gait.
7. Assess the patient's level of consciousness and determine if they are awake, alert, drowsy, lethargic, stuporous, or comatose (description of each on page 791 of your textbook).
8. Assess the patient's memory
a. Immediately- Provide the patient with a list of three objects and ask then to repeat the list correctly after 5 minutes.
b. Recent- Ask the patient a well-known current even to check recent memory. c. Remote- this test determines the patient's ability to recall their history such as their birthdate, names of children, first job, or hometown.
9. The provider continues with the neurologic examination and you may be asked to assist the provider. The provider checks the patient for the following:
a. Motor function- evaluation of the muscles of the body, symmetry of the muscles (right and left), and strength of the muscles.
b. Sensory function- pain (light prick of skin with a pin), touch (cotton ball to skin), vibrations of a tuning fork, and any others the provider deems necessary.
c. Cerebellar function- functioning of the cerebellum.
i. Finger-to-nose (FTN) Test- have patient alternate from pointing to their nose to the examiners finger while the examiner continues to move the location of their finger.
ii. Romberg test- have patient stand still with their feet together and arms straight at sides. Do they lose their balance?
iii. Heel-to-shin (HTS) test- have patient run heel of foot along the shin of opposite leg then repeat with other leg.
d. Reflex Function
e. Gait and Stance
10. Upon completion of the examination/assessment, record the procedure and findings in the patient's medical record.
