Case Study - Cognitive Interventions in Psychiatric Nursing

A 34-year-old man is voluntarily seeking treatment for severe obsessive-compulsive disorder. Currently, he is experiencing an exacerbation of obsessive thoughts that he has contracted HIV because he kissed his best friend's girlfriend during a recent holiday party. He has been engaged in excessive oral hygiene activities (mouth washing and brushing his teeth up to 37 times a day) to the point that he has compromised his oral mucous membranes and as a result has developed an infection on his oral tissue. In working with the patient, the Registered Nurse (RN) uses a cognitive behavioral therapy (CBT) approach with a plan to alter the patient's distorted beliefs by replacing the negative, inaccurate beliefs of contracting HIV and changing the compulsive lip- and mouth-washing behavior by rewarding more acceptable behaviors (Learning Objectives 2, 4, & 5).
