Don't just make it up off the top of your head (cite where you got you interventions w/page). Same for the implementations.

For Evaluation: make up a story- did it work? or not? How do you know (what was an appropriate criteria? if you treated pain, you should do a pain assessment. If you comforted her, she should say she is calmer maybe etc).
