Case Study - Mr G
Mr. G is a new resident in your LTC facility. Prior to visiting with him for a new resident interview, you have obtained the following information from Mr. G's admission records.
Mr G is 87 years old and has been diabetic x 25+ years. He is on the following medications for his diabetes: metformin 500mg BID and glyburide 60mg OD, Mr G's most recent HgA1C was 6.0. His weight is stable and he has a good appetite.

Mr. G doesn't like regular coffee and instead drinks decaf coffee. Mr. G does not like soups other than cream of mushroom and he prefers cold cereal for breakfast instead of hot cereals. Mr. G has also been drinking an average of 1.5 bottles or Boost Plus Calories at home for afternoon snack the past year or so and prefers the strawberry flavor.

One year later Mr. G returns to your LTC facility after a brief hospital stay for a stroke. Due to a new diagnosis of dysphagia, Mr. G now has a G-tube for feeding and comes back to your facility on the following feeding regimen
Resource Diabetic 75 ml/hr which runs for a total of 16hrs. During the 16hrs that his tube feed runs, he also receives a flush of 120mL of water every 4hrs, plus some extra water to flush down medications also given to him via the tube.
