The client has a history of Crohn's disease with intermittent gastritis. She also had surgery for an ileostomy six months ago. Ms. Lieberman's vital signs were stable but slowly getting worse as she complained of a pain scale of 6/10. Her baseline temperature was 98.8 Fahrenheit but increased to 101.8 Fahrenheit, pulse 114, respiration rate 22, blood pressure 100/60, and oxygen saturation 95%. Her hemoglobin is 7mg/dl, and her hematocrit is 21%. Upon arrival at the emergency room, a trace of serosanguineous effluent was present in her ostomy bag. She also had one unit of packed RBCs and part of the second bag infused via IV. And it was also noticed that her skin has been pale, warm, and dry to the touch.
