Patient: 12-year-old Latino male who presents with his mother
- CC: " I can't catch my breath for 3 months; I have asthma like my friend"
- HPI: Shortness of breath with activity such as playing ball or kicking a soccer ball associated
with coughing. Denies symptoms at rest or with sleep. Denies syncope, allergies, history of
asthma. Pain in center of chest with activity, sharp, without radiation, disappears with rest
within minutes. Pain began with onset of cold, cough, and low grade fever
- PE: Vital Signs: height and weight appropriate; Pulse: 102 RR: 24 Temp: 98.8 BP: 138/90
Appears well, in no acute distress
Lungs: mild inspiratory wheeze, mild intermittent hacking cough; PO 95%
Heart: Gr II/VI holosystolic murmur, low pitched heard throughout chest; pulses
intact, no cyanosis, capillary refill < 2 seconds
Rest of exam not contributory
- Initial Management: Albuterol nebulizer: chest did not clear
