You are reviewing a Roux-en-Y gastric bypass revision due to weight regain as the primary procedure. During the primary procedure, the surgeon made an inadvertent enterotomy to which enteric contents leaked into the abdominal organ/space. The enterotomy was repaired and the abdominal cavity was thoroughly washed and drained. The remainder of the procedure was uneventful. The patient was discharged on postoperative day 2. On postoperative day 15, the patient was admitted due to leukocytosis and pain at the incision site. Imaging revealed an abscess in the rectus abdominal muscle at the incision site and the patient was treated with IV antibiotics. A Deep Incisional SSI is assigned to the case.
