Case Study
George is a 23-year-old male, who lives with his girlfriend. He has history of depression, has started trials of multiple antidepressants in the past and is currently on Wellbutrin which was switched from Effexor recently.
George presents to emergency department (ED) for concerns of safety, and thoughts about hurting himself. George has made several statements that nothing ever helps him feel better and he has no reason to live. George has been reporting hopelessness and worthlessness, he is dysphoric and tearful on exam, and does not 'see a light at the end of tunnel.' He reports his symptoms of depression started in high school, he is not able to stressors, thinks that he is 'not made for this world,' and he started crying. States he has tried Prozac, Lexapro, Effexor and is now on Wellbutrin. George has been seeing a therapist, which he does not believe has been helpful. He continues to state that he does everything people tell him to do but his life is still miserable. He reports having no energy, no motivation, and anhedonia. George states he has trouble with sleep and appetite. He denies symptoms of psychosis, he denies a history of bipolar disorder but he does report irritability, anger, report sometimes 'his mind plays tricks on him.' He reports daily marijuana use and denies other illicit drug use. Denies physical, or sexual abuse. Currently unemployed. Denies previous suicide attempts.
