Reword In this particular instance, the licensed practical nurse did not notice the worsening of the patient and did not take the proper measures, despite the fact that the patient had previously expressed concerns. The licensed practical nurse was not qualified to monitor and assess the patient's status, nor did she have the proper supervision in this scenario. Additionally, the patient did not receive enough care. As a result, the charge nurse as well as the LPN share the ultimate responsibility for the inability to notice and react appropriately to changes in the patient's condition. As a consequence of this, I would place liability for this incident squarely on both the shoulders of the charge nurse and the LPN.
