A 45-year-old woman presents to you with neck pain and sweating. For the past week she has had myalgias, sore throat, and fatigue. She tells you that yesterday she noted sharp pain in the front of her neck. Her medical history is notable for osteoarthritis and right-knee replacement. She takes no medications, does not smoke, drink alcohol excessively, or use illicit drugs. She works as an administrative assistant for an investment bank. Her review of systems is notable for heat intolerance and mild anxiety. Her vital signs are: temperature 38.5 °C, heart rate 105 beats/minute, blood pressure 117/65 mm Hg, respiratory rate 16 breaths/minute, and oxygen saturation 98% on room air. Her examination is notable for exquisite tenderness over her thyroid gland without palpable nodules. Her thyroid-stimulating hormone (TSH) level is 0.1 microU/mL (normal range, 0.5-5 mU/L), and her free thyroxine (T4) is 15 ng/dL (normal range, 0.9-2.3 ng/dL).
