Why does ab now clinically develop hypothyroidism

AB is a 47-year-old female with a long history of mild obesity and hypothyroidism. Previous evaluation of her thyroid condition was consistent with primary hypothyroidism and she had been maintained on levothyroxine for the past 5 years. (Levothyroxine is primarily renally excreted and liver metabolized.) Recently AB has felt tired, cold, and constipated-all of the symptoms that she recalls from her initial diagnosis 10 years ago. She has been compliant with her medication and has not added any new medications or herbal supplements. She mentions that she has been taking her pill with dinner so that she does not forget to take it.

Medication

Levothyroxine, 100 mcg once daily

Physical Examination

Height: 5′10″; weight: 161 lb
Thinning of the lateral eyebrows
Delayed deep tendon reflexes
Mild non-pitting edema of the extremities
No other focal findings noted

Labs

Serum creatinine: 1.0 mg/dL

Thyroid-stimulating hormone: 6.2 mIU/L (normal: 3.5-5 mIU/L)

Renal and liver functions: normal
