For this task, you are required to write a short survey and get the survey completed by 4 families to determine their views and ideas on participation in the service.
Step 1 - Get permission from your supervisor to complete this activity at the service.
Write a short explanation/introduction to include in your survey which outlines the surveys purpose and contains a reassurance that the responses will remain anonymous.
Step 2 - Prepare your questions. Do not make your survey too long - try to stick to around six or eight questions.
Think about the information you are seeking - keep your questions focused. You can use different types of questions to achieve different outcomes.
Number each question so that it can be easily identified for discussion.
You need to ask questions relating to the following:
- What parents know about their existing options in relation to participation within the service?
- What information would they like to receive about their child?
- What they would like to see offered - you may like to give some ideas/options and/or ask for ideas.
Remember the easier your survey is to complete, the more likely it will be that you will get a high completion rate.
Show your survey to your supervisor and use their feedback to improve your survey.
Step 3 - Submit your survey template to your Director/workplace supervisor for final approval. Discuss distribution and collection strategies.
Step 4 - Determine the number of surveys you will distribute, as well as the method of distribution and collection. It is best to ask parents to complete the survey at the service rather than taking it home.
Step 5 - Distribute the number of surveys as discussed with your Director/workplace supervisor. Ensure you collect 4 completed surveys.
Step 6 - Analyse the results and prepare a short report n your findings. Make sure you answer the following questions in your report.
- What were the outcomes?
- What suggestions/ideas did parents offer?
- What recommendations would you make as a result of this survey?
Comment on the process you have undertaken and what you have gained from the experience.
Provide a copy of your report to your supervisor.
