Mary is a 20-year-old college student who presents to the office for evaluation of "trouble hearing" for the past week. Patient is sitting upright, wearing ear buds and talking on her cell phone. Patient states that three weeks ago she was treated for an ear infection while at home for spring break for which she reported "having a strong cough, a lot of congestion and a low-grade fever" which has sense resolved but ear pain has "not gone away". Patient states she has "noticed a small amount of yellow discharge" and a feeling of "fullness" in her right ear that she describes as constant and has been getting worse since she got back from her flight to school. Patient describes the pain as a 6 out of 10. Patient admits to using Q-tips to clean her ears but has not had any success and denies any other treatments. Patient denies any new injury or trauma but did state she has had history of several ear infections in the past. Patient states she remember getting ear infections since she was 10 years old. Patient states she uses her "ear buds" nearly everyday to listen to music. what is her diagnosis?
