Rewrite Therapy note: Ms Jazmun Waters is a 30-year-old AAF who presents for a therapy session via telehealth. The patient consented to telehealth. A therapeutic relationship is maintained, and the client is encouraged to express her thoughts and feelings. She reports she is 19 weeks and her partner is very supportive. She reports she is doing fine and taking her medications as prescribed. She reports breathing and meditation exercises are helping her cope with depression and anxiety. Today we discussed journaling of thoughts, emotions and feelings to help with reflection. She was encouraged to utilize learned coping skills, and She verbalized understanding. She denies any illicit drug use or ETOH abuse and denies current SI/HI/AH/VH/. Denied maniac/euphoric or elevated mood, impulsivity, flights of ideas, grandiosity, and increased energy. or any other psychotic spectrum of symptoms. The client is making progress towards his goal. Will follow up in 1 week.
