Rewrite: Mr. Hawkins is a 30 year old AAM who presents for follow up medication management. He complains that he got only one pill of naltrexone from his pharmacy which i established was a mistake.He brought up new information stating that he has been on olanzepine and he want to continue with his medication. Of note client was not forthcoming with his psychiatric history during his initial evaluation.When prompted further he disclosed he has been getting his medications from Washington DC.Client was requested to fill a ROI form so that we can request a list of his medications.We will follow up in a weeks time.
