Choose one client from the clip then fill in the following table to provide examples of how you assisted in the assessment of client risk for one client.   

Answer the following questions: 

  1. Who participated in the assessment process? How were everyone's roles and responsibilities described to the client?
  2. How did the nurse confirm with the client about the doctor's assessment requirements? 
  3. How was the client encouraged to participate in the assessment process?
  4. How was the assessment process explained to the client? Why was this important?
  5. What communication skills did the nurse demonstrate in the second approach in the video clip? How did he demonstrate a supportive, encouraging, respectful manner? Include some examples of his conversation to support your answer.
  6. In what way was permission for the assessment process gained from the client?
  7. What tools were used to assess client risk
  8. What strategies were used to minimise discomfort to the client and to maximise their participation?
  9. What information was gathered from reliable sources for example the individual care plan, medical records, measurements, and health professionals? How did this assist in the assessment process?
  10. Were there any issues or concerns identified that required referral to another person - for example the supervisor or a health professional. How was this done?
  11. Were there any lifestyle issues identified that affected their level of risk? What were these?
