Question

The patient is a 71-year-old male who came into the hospital with a cough and sputum that had been getting progressively worse for over a week. The patient was diagnosed with COPD and Acute Pulmonary Embolus (CT scan). The patient currently lives at home with his family before coming into the hospital. The patient has a past medical history of Chronic back pain secondary to spinal stenosis, hypertension, type 2 diabetes, Obstructive sleep apnea, CKD (chronic kidney disease), GERD (Gastroesophageal Reflux Disease), BPH (Benign Prostatic Hyperplasia), mood disorder, Fibromyalgia. Patient allergies are NSAIDs, SERTRA, Quetiapine, and IV Dye. The patient does experience pain and has depression and anxiety. subjective and objective data from this in sentence formate.
