A 60 year old white female was admitted with dyspnea and cough. Review of systems reveals fevers, chills, cough ( sometimes productive) and dyspnea (worse than baseline). Chest X-ray showed focal consolidation in the right lower lobe, suggestive of pneumonia. In the coders review of the record, several sign and symptoms of pneumonia are documented by attending physician but the attending physician list dyspnea as the diagnosis.
