YJ, a 22-year-old G0P0, presents with chief complaints of vaginal itching and a vaginal discharge that has a bad smell. She also complains of dysuria and dyspareunia. LNMP was 10 days ago.
Past Medical History: Tonsillectomy at age 5; Menarche at age 13 years; Never had a Pap smear
Medication(s): Daily vitamin; Birth control with low-ogestrel tablets
Family History: Mother: hypertension; Father: type 2 diabetes
Social History: Waitperson at local restaurant; Single; Nonsmoker; Denies illicit drug or alcohol use; Sexually active, multiple partners
Physical Examination: Height: 62 inch; weight: 110 lb; BMI: 20; blood pressure: 102/68; pulse: 76; temperature, 99.9 °F; Alert and oriented in mild acute distress; Cardiac: regular rate and rhythm, no murmurs auscultated; Pulmonary: clear to auscultation bilaterally; Abdomen: normoactive bowel sounds, soft, no masses palpated; Pelvic: external genitalia without lesions; cervix is erythematous with strawberry punctations; the cervix is friable. A yellowish/greenish/grayish bubbly vaginal discharge is present. Bimanual examination: tender uterus mobile, enlarged.
Labs: Urine: clear, yellow, few white cells present. Specific gravity 1; A normal saline wet mount reveals motile trichomonads. KOH is negative. 010
