A 42-year-old man had a SADI-S procedure at your facility for the purpose of weight loss. The procedure was captured on a Case Form for your site. On postoperative day 5, the patient went to the emergency room for persistent, worsening abdominal pain. He was found to have a rapid heart rate, a temperature of 103°F, and he appeared pale and diaphoretic. A CT revealed an intra-abdominal abscess from an anastomotic leak of enteric contents into the peritoneum. The patient deteriorated in the emergency department requiring intubation, fluid resuscitation, and antibiotics. Labs revealed a white blood cell count of 40.1, procalcitonin of 3.4, and a lactic acid level of 3.8. After a reoperation to repair the leak and washout the abdomen, the patient was transferred to the intensive care unit where he was started on continuous IV pressor therapy due to persistent systolic blood pressure readings below 90mmHg over several hours despite multiple isotonic fluid infusions.
