Which medical procedures and diagnoses for this patient and what are the codes related? He states he has had problems with throat tightness for the past two months. It has been gradually getting worse. He feels it is "like a claustrophobic reaction. Doctor's Assessment:

1. Throat tightness; this may be secondary to postnasal drainage or perhaps anxiety, etc.

2. Hematochezia. This is probably secondary to hemorrhoids; however , other possible etiologies cannot be excluded.

Plan: Flexible sigmoidoscopy to evaluate hematochezia and rule out polyps, malignancy, etc. He will start chlorpheniramine 8 mg sustained release one BID for postnasal drainage and throat tightness.
