Focusing on the Model for Improvement, a four-step approach to quality improvement consisting of identify, analyze, develop and test and implement, which is based on the Institute for Healthcare Improvement's (IHI) recommended model, discuss the first step of quality improvement which is to identify the problem.
Consider sample medications dispensing error concerns in a primary care office that have a negative impact on patients and your organization.
Answer the following QI initiatives questions:
1. What is the problem?
2. How do you know that it is a problem?
3. How frequently does it occur, or how long has it existed?
4. What are the effects of this problem on stakeholders and on the budget?
5. How will we know when it is resolved?
6. Is there measurable data that can be used to demonstrate the problem's existence?
7. What impact does that problem have on the 6 dimensions of quality?
8. What regulatory agencies are involved that are driving the need for improvements? What is their standard of quality regarding the problem?
8. For example, The Joint Commission (TJC) publishes its National Patient Safety Goals (NPSG) each year, which are meant to help organizations address issues of patient safety. One of the NPSGs is wrong site procedures. This includes a list of elements that must be included in the work processes to ensure that wrong site surgeries/procedures do not happen. If these untoward occurrences do happen, the event is reportable to TJC. Further review and action plans are expected as the organization works with TJC toward resolution of the quality concern/event.
