Patient: Chrissy J. is a 32 year old white female.
Presentation: Ms. J. presents to the community clinic today with concerns of intermittent diarrhea and bloating. Skin assessment reveals a stage IlI community acquired pressure ulcer on her right hip which has a surrounding area of erythema, is draining pus and has an unpleasant odor. Vital Signs: 101.5F, BP 142/88, HR 114, RR 18.
Anthropometrics: Height: 5'8" Weight: 118 pounds
Allergies: Sulfa
History: Ms. J. has paraplegia affecting her legs resulting from an automobile accident twelve years ago. She lives alone with her cat, Mr. Twinkles, in a small ranch home. She may legally drive her modified automobile but she does not go out often. Ms. J. required hospitalization two months ago for a fever. During the hospital stay, she experienced episodes of abdominal distention and diarrhea when drinking milk. Ms. J. become increasingly depressed about her status. Nurses reported that Ms. J. was angry and that they were concerned for their safety when providing care. Her oral intake is poor. She smokes 1.5 packs of cigarettes daily. Discharge diagnoses: urosepsis, lactose intolerance, and depression. A registered dietitian provided patient education on lactose free diet including removing milk from her diet.
Medications: TMP/SMZ DS twice daily for 10 days (she missed a few doses and has 4 tabs left). omeprazole 20 mg daily, ibuprofen 800 mg three times daily prn, citalopram 20 mg once daily, calcium carbonate 500 mg twice daily, vitamin D 800 IU once daily, bisacody| 10 mg daily.
Social/Family Dynamics: Ms. J. reports that her family does not offer valuable support in helping with daily life activities because they live two hours away. She talks by phone regularly with her sister who lives in Columbus. Ms. J. indicates that he has lost most of the friends that she had before the accident.
Support: A registered nurse makes homecare visits periodically as needed. A home health aide provides services twice a week.
