Problem

A large academic medical center has a communication practice in which nurses and physicians meet regularly to discuss and plan treatment goals and interventions for patients. This practice included joint rounds daily. Recently a very well-known physician was hired, bringing on physician assistant (PA) as well. This hire is seen as a clinical and public relations boost for the facility. This new physician made patient rounds with the PA, requesting that the nurses not participate, to "avoid delays." The physician will not respond to requests for communication by the nurses, directing them to "speak with my PA." While medical care has retained quality, the nurses feel that high quality nursing care is being obstructed due to the lack of communication and coordination. The nurse manager and clinical nurse specialist (CNS) have attempted to speak with the physician but have not achieved positive results.
