Problem

A 62-year-old man was started on digoxin 0.25 mg by mouth daily 4 weeks ago for congestive heart failure. He presents to the clinic today reporting moderate gastrointestinal upset and yellow-green halos around bright lights. Vital signs are all within normal limits. The patient says he is taking his medication as instructed and that he took the last dose 12 hours ago. A digoxin level returns at 1.5 ng/mL. Which actions would be most appropriate for this patient?
