Problem

Nakai is a 34-year-old Native American cis-gender male who was diagnosed with MS 2 years prior to this inpatient rehabilitation admission. His initial symptoms began approximately 3 years prior to this admission and consisted of diplopia, gait impairments, and problems with upper limb coordination. Nakai was admitted to the hospital because of worsening symptoms and an inability to complete basic self-care tasks such as feeding, grooming, dressing, and toileting. He is right hand dominant. His only PMH includes a fractured ankle at 16 y/o and an appendectomy when he was 18 y/o. Initial evaluation information: As the degrees of freedom (number of joints involved in the task) increased, tremors increase and functional control decreases. When the degrees of freedom were controlled (decreased) during treatment sessions, UE trajectories were smoother and resulted in an improved ability to interact with the environment and perform functional activities. When engaged in activities that challenged his postural stability, his tremors worsened. Observations to support this hypothesis included progressive worsening of tremors as he.
