Case: "Sara is an 18 year old, single woman who was brought in for counseling by her parents because of her "pre-occupation with food" has "gotten out of control". Although Sara says she is "deathly afraid of getting fat," Sara reports that she thinks about food most of the day and may think about big elaborate meals. She likes to cook for her family and buys many cookbooks. However, when she sits down to eat she eats very little. Her family is concerned because she "eats just like a little bird". What her family doesn't know is that after Sara eats she feels compelled to rush off to the gym to work out as soon as she can. Her workouts can last 2-3 hours. This pattern has persisted for the past 10 months. Sara has been losing weight and is now 98 pounds when her expected weight should be 125 pounds. Sara reports that she is very afraid that she is still "too fat" and when she looks in a mirror she grimaces at those parts of herself that are "still too big". She has also been caught 3 times for shoplifting in the past 2 years and she complains of mood swings. Sara also has a low heart rate, dry skin, intolerance to cold, and she reports symptoms of amenorrhea.
