Problem

FG is a 56 year old Hispanic female with Type 2 DM for 6 years. She likes to "eat what she wants" and admits to not following her diet. Her recent HbA1c came back at 9.0% , which is an increase from 7.9% over the past 3 months. She has been on Metformin for 6 months. Her weight is up 15 pounds and she now weighs 265 pounds, is not exercising, and is working 2 jobs on a daily basis. She is a smoker.
Her medications include:

1. Metformin 1000 mg bid
2. Hydrochlorothiazide 25 mg daily

What process would you apply in adjusting the patients diabetes medications?
