A 55-year-old man of Asian origin presents to a small rural hospital 50 miles from the nearest city with a new onset of chest pain. The pain is severe, located in the center of his chest and radiates into his left arm. The pain has come on suddenly and the man reports feeling very sick, nauseous, and sweaty. It has lasted more than an hour and at one point, the man thought he was going to die.
