Problem

This is a 76-year-old male patient admitted to the hospital with intractable pain secondary to stage IV prostate cancer with osseous mets to the lumbar spine. Upon admission, MRI of the spine was completed and worsening metastatic disease was noted. The patient's pain medication regimen on admission was morphine sulfate extended release (MS contin) 30mg PO BID and Morphine sulfate immediate release (MSIR) 5mg PO every 4 hours as needed for breakthrough pain. The palliative care team has been consulted and will see the patient tomorrow. In the meantime to control pain the physician has ordered to increase MS contin to 45mg PO BID and morphine 2mg IVP every 3 hours as needed for breakthrough pain. The patient receives his first dose of MS contin 45 mg at 1830. At 2030 he notes lumbar back pain of 8/10 and request PRN pain medication. (i) Document steps you will take before administering the PRN medication. What would you do if there is a finding which contraindicates giving the medication? (ii) Document a pain assessment and sedation assessment. (iii) Note the half-life of MS contin, when does it peak? (iv) What drug should be available for this patient in?
