The resident is 89 yrs old with advanced dementia. Her labs are currently within normal limits and she has not significant medical history other than dementia. She is currently consuming only 25% of her meals and is on a general diet. She is 5'2" and currently 95#. Two months ago she was 118#. After a family meeting with the multidisciplinary team, the patient will start enteral nutrition support.
What would the nutrition care plan be (nutrition diagnosis, interventions, monitoring and evaluation) using the eNCPT standard language.
Your nutrition interventions should include enteral nutrition support recommendations (include the formula name, rate at which you would run your feeding, the amount of calories, protein, and water your enteral recommendation would provide. Please also calculate free water flushes if needed).
