She is currently consuming only 25% of her meals and is on a general diet. She is 5'2" and currently 95#. Two months ago she was 118#. After a family meeting with the multidisciplinary team, the patient will start enteral nutrition support.
What would the nutrition care plan be (nutrition diagnosis, interventions, monitoring and evaluation) using the eNCPT standard language.
