A 28 year old female presents for c/o low back pain especially in the buttocks that is worse at night. Some relief taking OTC NSAIDs. Better with activity, worse with prolonged sitting or lying down. Occasional pain and swelling in the fingers. Has also been experiencing Achilles tendinitis. Her xray shows signs of sacroillitis. Based on this information, what would be your primary differential diagnosis?
