In terms of strengths, this study benefitted from adequate sample size and methodological rigor. The study evaluated 185 adult patients in an urban ED setting. Furthermore, this study utilized both quantitative and qualitative data collection methods to gain insight into various aspects related to the effectiveness of nurse-initiated protocols. The study also accounted for variables such as nonpharmacological analgesics and patient-administered analgesics prior to ED check-in. This study, combined with similar research, might soon allow for the broader adoption of nurse-initiated protocols in the ED. Despite the strengths reviewed, there were several limitations.
The study was conducted at a single location, and the study participants were of higher socioeconomic backgrounds, so it may not be applicable to other environments (Boessio dos Santos et al., 2021). Another limiting factor was the lack of randomization when allocating patients to the intervention and control groups. The patients were allowed to choose which treatment group they entered. Additionally, the time frame of this study was from April to September.
This could have affected the sample diversity by excluding patients that suffer from a variety of disease processes typically exacerbated by seasonal changes. When evaluating and treating pain, it is important to note the nature of pain. The authors did not examine this feature, which may have tainted the results. For example, most nurse-initiated protocols utilize the use of non-narcotic analgesics. These types of analgesics may not be an effective treatment modality when treating chronic pain versus acute pain.
