Case Scenario B Hospital A oversees an urgent care center about ten miles away. Patient John Brown is seen at the center with a fever, dysuria, urgency and abdominal pain. After being examined by the medical staff a urine analysis (UA) was performed. Even though most of the specimens from the urgent care center are sent to the main hospital laboratory for testing, the UA was performed at the urgent care center by one of the staff using a small automated analyzer. The results included elevated leukocyte esterase and nitrite values, and 5-10 WBC per high power field seen microscopically.
(a) Given the symptoms and abnormal U/A results, what is the most likely clinical diagnosis for John Brown?
(b) What were the advantages of performing the test at the urgent care center rather than sending it to the main laboratory?
(c) Describe the regulations for performing this category of testing (outside of the setting of the main lab). Your answer should refer to the licensing requirements for New York State, staff competencies and quality control requirements.
