Case Study
Sami Gordon-Cruz (he, him, his) is a 6-year-old Argentinian boy who witnessed the murder of his father, Manuel, 6 months ago. Manuel was shot multiple times during a robbery in the bodega where he worked down the street. Since the murder, Sami has had trouble sleeping, often cries, and has been refusing to leave the house. His mother, Adelina (she, her, hers), notes that Sami regularly complains that his tummy hurts, is startled by any sudden or loud noises, and constantly wants to be held. He has a poor appetite, and he doesn't sleep well.
The family is Catholic. Adelina says that she has told Sami many times that his papi is in heaven, and they pray for him every day. Sami does not appear to be comforted by this.
Adelina tells you that her family came to the U.S. about 4 years ago and are living here without authorization. Some of her cousins and their families live in the area, but her closest relatives are in Argentina, in particular the grandmother who raised her. Adelina and Sami speak English, although the family primarily speaks their native Spanish and Quechua at home and in the community.
From your perspective as Sami's psychiatric nurse practitioner, answer the following questions.
1. What type of psychotherapy do you recommend for Sami? Provide a rationale. How would you explain to Adelina how this intervention could help her son?
2. Identify a case conceptualization based on cognitive behavioral therapy for this case.
3. What level of care would you recommend for Sami? How often would you want to see him? Are there any recommendations you would make for the family as a whole?
4. Sami has state Medicaid insurance, which you do not take. How do you address this with the billing department and office manager?
