Question: P.T. is a 37-year-old, single woman, Gravida 3 Para 2, 28 weeks gestation. She arrives to the Labor and Delivery unit complaining of lower back pain and urinary frequency. She has a history of gonorrhea (was treated), and smokes cigarettes. She has not received prenatal care, nor is she accompanied by a support person. Based on the information, identify two most likely diagnosis for her. Name risk factors putting P.T. at risk for complications in this pregnancy? What other assessments, including laboratory tests, are needed to accurately treat this patient? P.T. is found to be contracting every 2-3 minutes , Fetal heart rate of 145 with moderate variability. Her cervix is dilated to 2cm, 80% effaced, and -1 station. An 18 gauge IV is started, and Lactated Ringers are ordered and infusing at 125cc/hr. Complete blood count and urinalysis are within normal limits. The fetal fibronectin (FFN) is positive. Urine drug screen is positive for heroin. The GBS and other cultures are pending.
