J.R is a 33 y/o male presenting with weight loss (unintentional), drinking and eating a lot, fatigue and urinary frequency for the past 6 months. He is active with his landscaping job and has not seen a provider for medical care for over 8 years.
Physical exam: BP 110/70, HR 68, RR 16 SpO2 98%, BMI 19
WD male, NAD, Alert/oriented x 3. HEENT WNL, Heart S1S2 RRR No MRGs. Lungs CTA, Abdomen soft, nontender, no organomegaly, BS + x4 normoactive. No CVA-T. Monofilament test and 128 mhz tuning fork test reduced bilaterally peripheral.
