Question - The learning scenario focuses on the gastrointestinal and GI assessment.
Sophie Ricci, 15-year-old female, presenting with a chief complaint of abdominal pain.
She is alert and responsive but appears anxious. Her mother is by her side.
1. What tools will be necessary to have ready prior to beginning the focused assessment on the client (any questionnaires, standardized assessment tools, flowsheets, etc.)?
2. What tools will be required to gather to perform the vital signs, pain and cardio-resp assessments? In what order should we perform these assessments?
3. What subjective assessment questions are you going to ask the client?
Regarding their health history (most specifically of cardio-resp problems)
Regarding their current health problem (how is she feeling now? Any pain, difficulty breathing, etc.)
Regarding any red flags (warning signs) that might be present for this client (ex: shortness of breath, feeling very tired, difficulty performing normal daily activities, worsening symptoms despite use of medication, etc.)
4. What physical assessment activities do you anticipate performing? Are there any considerations for the client's age or situation and what can we expect from the results?
