Case Study: A 14-year-old adopted African American female to male presented to inpatient crisis complaining that his parents don't understand him. The patient has increased anxiety, depression, and suicidal ideation due to his parent's acceptance. Patient said that he wanted to kill himself with the plan to run into upcoming traffic. Has a history of self-harm, and his last self-harm was 2 days ago. Reported increased appetite and gaining 10 lbs in a month. Reported poor concentration and fear of failing 2 classes. Currently taking Prozac 40mg for anxiety and depression. Patient reported that he hates his breast and wraps it to flat. The patient wants to wear boys' clothes, use boys' bathroom and participate in boys' activities at school. Reported that he is attracted to females and heterosexuals. Distress at developing secondary sex characteristics associated with females. Had family therapy at home for 6 months, and treatment ended 4 months ago. Reported smoking 5 cigarettes per day. Denied other substance use. The patient was adopted at age 5 and has been in foster care since age 3. The patient had neglect and physical abuse by the biological mother from birth till 3 years old.
