Assignment -

Instructions: You will be helping a family member, friend or loved one write up an advance directive using the Vermont Advance Directive Short Form that you received in class.

Recommend doing this assignment with someone you could potentially have to make decisions for in the future (for example a parent, partner, spouse, close friend or family member) because you may get new insight about their wishes. But they do not have to choose you as their agent.

Help them think through and guide them with information to complete sections 1-3 of the form:

Part 1: Who do they want to be their healthcare agent if they cannot make decisions on their own?
Part 2: What are their goals for medical care? What is important to them to be able experience in life?
Part 3: Do they want any limitations on medical treatment in certain situations? Do they understand the potential benefits and burdens of these treatments?

Refer to the information in the lecture slides for information and tips on talking through each of the sections.

Include a copy of the completed form (you can bring it to class or make a copy and upload to Canvas) and write a 2-page double-spaced paper on what the experience was like for you and the other person (around 500 words).

Guiding questions for reflection on the experience (you do not have to address all of them):

What were their thoughts and questions about advance care planning? What information were you able to provide?
Was there any resistance on their part (or yours) to working on their advance care plan together?
Was there anything surprising to you about their wishes? If so, what?
Did they have wishes that were different than what you would have chosen?
Did any challenging emotions or uncomfortable moments come up in the process? How did you work through?
