Problem

Santiago is an 82 year old man who was in reasonable good health until about a year ago. Last year he suffered multiple TIAs followed by short hospitalizations. After discharge his physician prescribed outpatient speech therapy to improve cognition, physical therapy and occupational therapy were prescribed for gait training and ADL improvement, respectively. Santiago has been rather independent all of his life and doesn't want to be a burden on anyone in the event his health continues to decline. He also doesn't want any heroic measures taken if he suffers a hearth attack or any other life threatening event.
