Description

Simon is a complex case manager for a large health care system. His job is to help individuals with chronic conditions like CHF, DM, MS, COPD, and RA manage their conditions. He keeps expenses related to diagnostic tests, physician visits, DME, home care, ED visits, and hospitalizations.

Recently, he's noticed that several of his clients with comorbid conditions of CHF and COPD have been visiting the ED more frequently and have been readmitted to the hospital within 10 to 20 days of discharge.
