Case Study: Mr Patel an 82-year-old male, was admitted yesterday to the medical ward for observation and pain management of his right (R) shoulder following a mechanical fall. Mr Patel tripped on a step at his daughter's home and landed heavily on a near by bench, which broke his fall. He has sustained heavy bruising and swelling to his right (R) shoulder and upper arm.
Mr Patel is a Hindi-speaking man who migrated to Australia from India in 1998 with his wife and three children. He is from a culturally and linguistically diverse background. He has been married for 58 years and has three children all of whom live close by.
During handover this morning Mr Patel stated that his was very embarrassed about falling and causing so much concern and work for everyone. He believes in Karma and states "I must have done something bad and now destiny is paying me back".
Overnight he received adequate pain relief and his last dose was at 0600.
You are a student Registered Nurse on your INC2 PEP placement. You and your buddy nurse, a Registered Nurse, have been providing care for Mr Patel during this morning shift. It is now 12:00hrs and you are checking on Mr Patel and tidying up his bedside table before lunch.
During this time you have noticed that Mr Patel is:
- Quiet and withdrawn,
- Responses are short and abrupt.
- He is guarding his right arm and expressing concern about you touching it.
- Facial expressions show concern and grimacing.
- When you ask him about his pain, he states 'he has no pain'.
- You exit his room reflecting on how his behaviour has changed from this morning. You recall earlier in the morning how he was smiling whilst chatting with his wife and engaging with you in conversation.
