Case: Mr. Johnson is a 75-year old African American male who is readmitted to the hospital for heart failure for the fourth time in the past year. He has a history of atrial fibrillation, type 2 diabetes with peripheral neuropathy. His other diagnoses include hypertension, low ejection fraction, and hyperlipidemia. A year ago he had a suspected transient ischemic attack with no recurrence. His wife of 56 years just died 6 months ago after a massive stroke. He has a daughter who live approximately 45 minutes away. She has 2 children ages 7 and 11, and a full-time job as a CPA (certified public accountant). Mr. Johnson's medications include: warfarin, aspirin, Lasix, glyburide, gabapentin, Elavil, and atorvastatin.
