Case Study: Mrs Tim is a 64-year-old woman who has a known diagnosis of schizoaffective disorder who is on level 2 observation and under section 2 of the Mental Health Act. She is currently paranoid and psychotic. She is admitted into the older female adult mental health unit of the full Brookes Centre, Cherwell ward. She has a long history of some physical health challenges such as COPD and severe arthritis. On admission, Mrs Tim was presented with symptoms of persecutory delusion and disoriented thought. The state of her mental and physical health poses some risk such as poor nutritional intake, attack on care staff, self-harm, no capacity with regards to agreeing to take her medications.
I chose to work with Mrs Tim for my episode of care in agreement with my supervisor because she has been in the ward from the beginning of my placement and she appears to be one of the most challenging patients to deal with in Cherwell ward due to her identified risks which includes, verbal aggression, racial and physical abuse. She attacks Care staff who try to support her, she believes they are poisoning her and therefore refuses food and fluid most of the time.
